Forms
Below is a list of USF Undergraduate forms in the Curriculog Proposal System.
Course Change Form
This form is used to propose changes to an existing undergraduate course. The Short Form is for changes to: Course Number, Course Description, Course Requisites, Grading Mode, Schedule Type, and Restrictions only.
Course New Form
This form is used to propose a new undergraduate course.
Course or Attribute Discontinue Form
This form is used to do either of the following: 1. To discontinue an existing undergraduate course at USF. 2. To remove one or more attributes from an existing undergraduate course at USF.
Course Gen Ed Recertification Form
This form is used to propose a new certification for a General Education course.
Program Change Form
This form is used to propose changes to an existing undergraduate program (Major, Concentration, Minor, or Certificate).
Program Discontinue Form
This form is used to discontinue an existing undergraduate program (Major, Concentration, Minor, or Certificate).
Program New Certificate Under Existing CIP
This form is used to propose a new undergraduate program Certificate under an existing CIP code.
Program New Concentration Under Existing CIP
This form is used to propose a new undergraduate program Concentration under an existing CIP code.
Program New Major Under Existing CIP
This form is used to propose a new undergraduate program Major under an existing CIP code.
Program New Minor Under Existing CIP
This form is used to propose a new undergraduate program Minor under an existing CIP code.
