Academic Advising Survey

The Academic Advising Survey is a platform for you to express your thoughts about your recent academic advising experience.


ARC Petition

The Academic Regulations Committee (ARC) Petition is used to request an exception to University policy.
 
Please refer to the guidelines for additional information and contact your advisor with any questions or concerns.


College of Education Petition

The College of Education Petition is used to request an exception to College of Education policy.

You should contact your advisor about the policy and whether a petition is warranted.


Course Substitution Form

The Course Substitution Form allows students to request an evaluation of a transferred course to determine if it is equivalent to a USF course(s).

Please schedule an appointment with an academic advisor in the College of Education to receive the form and initiate the process.


Declaration of Major Form

This form allows students to declare a major in the College of Education as a Pre-Admitted student. Please be aware that the Declaration of Major is not the same as a formal application to the College of Education.

Declaration of Minor Form

This allows students to declare a minor in the College of Education. Students earning a minor from the College of Education are not required to earn a major in the College of Education.

Time Limitation Form

The  allows former students returning and transfer students to request an evaluation of USF courses and other institution courses taken over five years but less than 10 years ago to see if it is an equivalent to a current USF course(s). This form is used when you want older courses applied towards your degree.
